Prob transmission. Shift forks are bound up. Or maybe the shift drum was put in wrong.. Could be the return spring broke or detent wasn't put in right.

I would expect the worst. 86 transmission parts are expensive

It doesn't shift into first when it clicks down?

I've seen people put a bolt that was too long on the shift lever/shaft, hitting the frame making it not click down. But that can't be it cause it clicks down.

Either way, if its clean, and can be bought cheap, it might be worth it. Check for welds and case damage. If it's good your buddy can use the transmission issue as a big bargaining chip.
Say motor needs to be pulled. Cases split. Prob new trams. Gaskets. Labor. Lots of time
